Where do i put my W-2s?

So, i filed my federal online, but not my state. I did them myself, and i want to correspondence them. Where on my state form do i attach my W-2 and with what?
(I didnt report State online because they charge $20 for it, and im only getting $15 spinal column. I only worked for 2 months)
