How does someone gain insurnace for expensive medical problems? Just curious.?

My Uncle Don have probably spent at smallest $2million on his medical problems in days gone by 12years. By the age of 35 he be 250pounds overweight and be a big smoker. He still lives one and the same lifestyle. He is getting surgery done that costs him $250,000 near insurance. What type of insurance covers associates next to expensive medical problems? Just curious.
