I go to an emergency room using my insurance card, purely approaching adjectives times they denied one of the claims the total cost be $ 22.00, I wasn't inform nearly it, it go to a collection and very soon it is on my credit report as extremely historic due, how I can encounter them to wipe sour of my credit report.

I would suggest you contact the claims department and ask for copies of the bills and notice sent to you...Something close to this happen to me but it turned out the address the things be sent to be opening wrong.
