Why are in attendance so abundant errors by companies at the moment?

It seem at the moment every time I enjoy interactions near a company the error rate is extremely illustrious.
Like insurance companies and bank.
My girlfrieind have call three times to her insurance company unfolding them they want to clear the mamogram because she is not a masculine wich be the principle for refusal to repay. They rewarded the PAP smear!
Each time they share her it have be fixed. 6 months in a minute.
This newly seem to be the norm any more an error or issue take multiple phone call. to fix.
Some of this is due to the outsourcing to India of the call upon service.They appear to hold a culture of nodding and doing nil or defer.
But it is growing ever worse.
What is cause this? .
Is it the MBA's that outnumber the workers outsourceing to inaffectual groups of late ringing bread registers and recitation the corporate boards how pious they are doing?

Most of these companies are top-heavy when it comes to compensation. The bosses engineer lots of money, departing little payroll beside which to conscript biddable entry and mid-level
employees--hence, incompetent idiots abound.
Mostly it's because information is save on multiple systems, as all right as "historical information", so within is some computer application somewhere that have her tabled as a mannish, and it continues to be propagated because near is no single "gold ingots source" of the truth that you can fix once and everything else is correct.

As far as insurance co's turn,I've be denied grant because of insufficiency of pre-approval, service wasn't contained by net,my deductable wasn't met, etc. In adjectives of those cases they be false, but its contained by the best interest of the insurance co NOT to settle up you,... ie they hold on to the money. I've hear nearly plentifully of others who've have denied claims even though it say they are covered within black & white on their policies. Honestly,I mull over its a shady tactic to stockpile the insurance co money,but its adjectives too adjectives.
